Incident overview

A blockage in the boiler system caused the fire inside the combustion chamber to burn out of the front of the boiler and into the mechanical room. The finned heat exchanger was found to be plugged with soot and carbon. A contributing factor to the fire was the proximity of the boiler to the combustible wall.
